Streszczenie

Wprowadzenie. Zapalenie przyzębia oraz cukrzyca typu 2 są chorobami przewlekłymi, które obecnie ze względu na powszechność występowania można uznać za choroby społeczne. Niewyrównana cukrzyca i stan zapalny tkanek przyzębia mają na siebie wzajemny wpływ. W badaniach zależności między zapaleniem przyzębia a chorobami ogólnoustrojowymi wykorzystuje się różne wskaźniki periodontologiczne, głównie CPI. W 2008 roku Offenbacher zaproponował nową skalę oceny stanu przyzębia opartą na wskaźniku krwawienia dziąseł i głębokości kieszonek.
Cel pracy. Ocena zależności między stopniem nasilenia zapalenia przyzębia a poziomem glikemii oraz stężeniem glikowanej hemoglobiny u pacjentów chorych na cukrzycę typu 2 z zastosowaniem wskaźnika CPI oraz skali według Offenbachera.
Materiał i metody. Badaniem objęto 84 pacjentów leczonych z powodu cukrzycy typu 2 w CSK MSWiA w Warszawie. U pacjentów określono stopień aktywności zapalenia przyzębia na podstawie skali Offenbachera oraz oceniono periodontologiczny wskaźnik CPI. Oznaczono również poziom glikemii na czczo i stężenie glikowanej hemoglobiny (HbA1c).
Wyniki. Mimo stwierdzonej w badanej grupie korelacji między stanem przyzębia (wyrażonym wskaźnikiem CPI) a stężeniem glukozy i HbA1c we krwi nie obserwowano takich zależności dla stopnia aktywności zapalenia przyzębia według skali Offenbachera.
Wnioski. Ocena przyzębia w skali Offenbachera, która jest bardziej czasochłonna w porównaniu ze skalą CPI nie sprawdziła się jako wskaźnik stosowany w badaniach zależności między chorobami przyzębia a stopniem kontroli cukrzycy.

Abstract

Background. Periodontitis and type II diabetes are chronic diseases which can be now considered social due to their widespread prevalence. Uncontrolled diabetes and periodontal tissue inflammation impact each other. In the studies on interrelationship between periodontitis and systemic diseases various periodontal indicators are used, mainly CPI. In 2008 Offenbacher proposed a new periodontal disease scale, based on the bleeding index and periodontal pockets depth.
Objectives. The assessment of the relationship between the severity of periodontitis and glycaemic and glycosylated hemoglobin levels in patients with type 2 diabetes by using CPI index and the Offenbacher’scale.
Material and Methods. The study included 84 patients diagnosed with type 2 diabetes who were treated in the Clinical Department of Internal Diseases, Endocrinology and Diabetology, Central Clinical Hospital of Ministry of Interior in Warsaw, Poland. The status of periodontal tissues was determined according to Offenbacher scale and Community Periodontal Index (CPI). Basic laboratory tests were performed, including fasting glycaemic level and glycosylated hemoglobin (HbA1c) concentration.
Results. Even though a significant correlation was found between both serum glucose and glycated hemoglobin level and periodontal activity according to CPI index, no such relationship was observed when the Offenbacher scale was used.
Conclusion. The assessment of periodontal status using Offenbacher scale is more laborious as compared to the CPI, and did not prove superior in the study on relationship between periodontal disease and type 2 diabetes.
