Streszczenie
Wprowadzenie. Twardzina układowa jest chorobą o podłożu autoimmunizacyjnym. Jej charakterystyczny obraz kliniczny jest wypadkową zmian włóknistych narządów. Punktem zainteresowania autorów pracy stały się objawy kliniczne chorób w obrębie jamy ustnej i twarzy.
Cel pracy. Ocena charakteru i częstości występowania patologii jamy ustnej typowej dla twardziny układowej, próba określenia subiektywnych odczuć i problemów stomatologicznych pacjentów chorych na twardzinę układową.
Materiał i metody. Badania przeprowadzono w Klinice Dermatologii Uniwersytetu Medycznego w Lublinie w grupie 24 pacjentek z postawionym rozpoznaniem twardziny układowej. Pacjentki zostały poddane badaniu przedmiotowemu oraz poproszone o wypełnienie ankiety dotyczącej indywidualnego przebiegu choroby. Oceniono występowanie zmian na błonie śluzowej jamy ustnej, wargach, języku i tkankach przyzębia. Grupę kontrolną stanowiły 24 zdrowe pacjentki zgłaszające się do Zakładu Stomatologii Zachowawczej Uniwersytetu Medycznego w Lublinie w celu leczenia stomatologicznego.
Wyniki. Zmiany chorobowe najczęściej występowały na języku. U 19 pacjentek był on wygładzony ze skróconym wędzidełkiem podjęzykowym. U 18 pacjentek zaobserwowano zanik czerwieni wargowej. U połowy badanych stwierdzono zwiększone napięcie błony śluzowej jamy ustnej. U 2 badanych chorych nie znaleziono w obrębie jamy ustnej żadnych zmian typowych dla twardziny, zmiany skórne występowały jednak w całej grupie pacjentek. Wśród najczęściej zgłaszanych problemów dotyczących umiejscowienia wewnątrzustnego znalazły się uczucie suchości oraz ograniczenie ruchomości języka. Klinicznie zdrowe przyzębie stwierdzono u połowy badanych.
Wnioski. Problemy stomatologiczne pacjentów chorych na twardzinę układową można określić jako znaczące. Postępujące zmiany mogą zwiększać ryzyko próchnicy i chorób przyzębia, a w konsekwencji prowadzić do szybkiej utraty zębów. Patologiczne włóknienie tkanek miękkich utrudnia leczenie stomatologiczne i korzystanie z uzupełnień protetycznych. Z wymienionych powodów ważna jest wczesna i trafna diagnoza oraz odpowiednie leczenie, a także skuteczna profilaktyka przeciwpróchnicowa.
Abstract
Background. Systemic sclerosis is a disease of autoimmune origin. Its characteristic clinical picture is a result of fibrotic changes in the bodily organs. Clinical manifestations of the disease within the mouth and face have aroused the interest of the authors of this article.
Objectives. The aim of the study was evaluation of the nature and incidence of oral pathological changes typical of systemic sclerosis; an attempt to determine the patients` subjective feelings about the disease and associated dental problems.
Material and Methods. The study was conducted in Dermatological Clinic of the Medical University of Lublin, Poland, and it included a group of 24 female patients diagnosed with systemic sclerosis. The patients were physically examined and asked to fill in a questionnaire concerning the individual course of the disease. The mucous membrane of the mouth, lips, tongue and periodontal tissue were of primary interest. Control group was made of 24 healthy patients admitted to Stomatological Clinic of the Medical University of Lublin for dental treatment.
Results. The tongue turned out to be the most frequent localization of pathological changes, in 19 of all the patients it was smooth and the sublingual frenulum was shortened. Thinning of the lips was observed in 18 of the studied patients. Half of the patients presented increased tension of the mucous membrane. In two of the investigated group no abnormal changes typical or systemic sclerosis were found, while skin changes were observed in all the studied patients. Dryness of the mouth and decreased tongue mobility were the most commonly reported symptoms concerning intraoral localization. Clinically healthy periodontal tissue was found in half of the examined patients.
Conclusion. Obtained information gives grounds to define dental problems in patients suffering from systemic sclerosis as significant. Progressive changes may increase the risk of dental caries and development of periodontal diseases, which may lead to untimely loss of teeth. Pathological fibrosis of soft tissues makes it difficult to carry out dental treatment and apply dental prostheses. Because of all the reasons mentioned above, early and proper diagnosing and appropriate treatment as well as successful anticaries prophylaxis are of the utmost importance.
